I wish they could get an actual guy to do Rito's voice because I feel that his female seiyuu, to put it nicely I guess, doesn't fit and is way too girly.

Since there are now a number of OVAs from To Love-Ru (original and Darkness), hopefully Sentai will pick them up as a batch; that might be economically possible. (I understand why just picking up one is not.)

Sentai did license the OVA Highschool of the Dead: Drifters of the Dead. I do not know if that was bundled with a manga.

You'll probably have to give it some time. With the way Lala has... changed, there might be some ethical issues with Rito making her his love interest.

I believe you win the prize, as it was indeed bundled with the (7th volume of the) manga.
