Eh... Don't remember a scene like this and you've used this show before so I have a lot of doubts, but I guess I'll still go with my first impression Dekomori Sanae from Love, Chunibyo & Other Delusions.

That's correct. Its from the "Depth of Field" shorts. I use each encyclopedia entry once, so when an anime has a bunch of shorts that are given separate entries, I'll generally end up using it several times. In this case, there are still the sequel series, some shorts from the sequel, and the movie, so I'll probably eventually use this show 3 more times once I get around to watching those (but not necessarily any time soon).

Correct, though I don't get the 14 year old part.... wait a minute. Holy shi-.. HE IS 14?! All these years and I always thought he was an adult. He is younger than Touma... You just completely destoyed my image of this dude. Though I guess it's not that weird considering there's a character like Komoe in this show.

Oh, yeah, the actual ages vs. apparent ages vs. behaviors/responsibilities thing is way out of whack in the whole franchise; in fact, I think its novels probably deserve at least some blame for that being ramped up in recent years.
