On Monster, was Australia's Siren Visual the only company to have released the entire series on disc, or have they done that elsewhere (in English)?

They haven't done it in the US at least, Viz released one DVD boxset of the first 26 epicodes or so. This is very weird because they actually dubbed the entire series and aired it on The Sci-Fi Channel. Instead of re-releasing the rest of the anime, though, they're republishing the manga under their Signature label.
