I just noticed that on the web site it said "Finally, Yoko Ishida sang three songs, including the Neon Genesis Evangelion opening theme song, and the Ah! My Goddess! opening theme." However, the part where Yoko Ishida sang the opening theme song for EVA is incorrect because it was Yoko Takahashi who sang EVA's opening theme song.

You may be right, who knows, but we're talking a news article from 5 years ago. This just isn't relevant anymore.

Not at Otakon. Singers can sing songs that are recorded by other singers...
