Here are a few...

Rebecca Forstadt (person #2225) = Rebecca Olkowski (person #5191). Olkowski is her married name, and she is sometimes found on the Internet under this name.

Haim Saban (person #7232) = Kussa Mahchi (person #23537). Kussa Mahchi is the pseudonym Saban used for his music credits in every anime dub from his company from the mid-90s on. Practically every mention of the name on the Internet indicates it is a pseudonym for him.

Samuel Vincent (person #3935) = Sam Kouth (person #7224). His own page here notes that he was born Samuel Vincent Khouth; clearly the latter is a pseudonym for him (and if you listen to the roles where he's credited under that name it definitely is him).

Shirley Millner (person #2455) = Shirley Milliner (person #66534). Obviously a misspelling; the former is clearly the right one.

I. Kaiji Tang (person#70445) (real name), Juicey Flannigan (person#104918) (alias) and Juciey Flannigan (person#104915) (erroneous spelling)
Tang himself has stated outright that “Juicey Flannigan” is his non-union handle.

II. Karen Neill (person#38529) (correct spelling) and Karen Neil (person#31630) (incorrect spelling)
Neill is consistently spelled with 2 ‘l’s in her last name. The credits for Sonic X (0:06) are no different.

III. Andrew Rannells (person#12007) (correct spelling) and Andrew Rannels (person#11999) (incorrect spelling)
Credits for Kirby (0:02) spell his last name with 2’l’s as usual.

IV. Wayne Sharpe (correct spelling) (person#7955) and Wayne Sharp (person#12031) (incorrect spelling).
Further down in Kirby: Right Back at Ya credits (0:10) is “Wayne Sharpe”, and Sharpe is how he spells his name on his own website.

V. Juli Erickson (person#44888) (correct spelling) and Julie Erickson (person#70204) (incorrect spelling)
An extensive tribute to her and Grant James at Dallas Video Fest demonstrates that “Juli” is how her first name is usually written, at least in the field of acting.

VI. Rodger Bumpass (person#14138) (correct spelling) and Roger Bumpass (person#13779) (incorrect spelling)
“Rodger” is how his first name is spelled, as demonstrated in the news by that DUI he got earlier this year. Despite being added first, any of the anime roles that might have been originally added under “Roger” have since been moved to “Rodger” accordingly.

VII. Gabe Khouth (person#16932) (correct spelling) and Gabe Kouth (person#16752) (incorrect spelling)
Gabe’s website and social media profiles are proof that “Khouth” is the correct spelling of his last name.

VIII. Ellyn Stern (person#3043) (correct spelling) and Ellen Stern (person#25330) (erroneous variation)
The ending credits of Mechanical Violator Hakaider list “Ellyn Stern” under “English Voice Cast”, not “Ellen”.

IX. Rebecca Forstadt (person#2255) (correct spelling and Rebbeca Forstadt (person#60581) (misspelling)
Straightforward misspelling.

X. Samara Naeymi (person#149755) (correct spelling) and Samara Nayemi (person#152607) (misspelling)
The spelling of Samara’s last name is verifiable via her Twitter, Youtube, etc.. The cast credits for the Pokemon Sun & Moon sneak peek dubbed episodes, however, misspelled it as “Nayemi.”

XI. Xanthe Huynh (person#69839) (correct spelling) and Xanthe Hyunh (person#104913) (misspelling)
Verifiable via social media.

XII. Brittney Karbowski (person#44595) (real name) and Brittany Karbowski (person#52380) (misspelling)
Verifiable via social media.

XIII. Vince Corazza (person#4120) (stage name) and Vincent Corazza (person#94007) (full name)
As shown by his official website, “Vincent” is his full first name, which in voiceover has been used in at least the show Braceface and the Kingdom Hearts video game series.

XIV. Catherine Cavadini (person#40114) (correct spelling) and Cathy Cavadini (person#99820) (variant)
Though she is more casually known and credited as “Cathy”, Cavadini’s first name was written as “Catherine” in both anime she’s been listed here for.

XV. FB Owens (person#137584) (credited name), Frederick B. Owens (person#56147) (full first name) and E.B. Owens (person#30491) (erroneous spelling)
“F.B. Owens” is regularly credited in the credits for 4Kids’ One Piece dub, as seen in this vid (variants include 0:08, 0:37, 1:09 and 7:21), therefore “E.B.” is a misspelling, likely due to bad video quality recording. “F.B. Owens” is also seen in 4Kids’ Teenage Mutant Ninja Turtles, and Gol D. Roger (One Piece) and Traximus (TMNT) are verifiable to be Frederick in both via Frederick himself on Twitter: https://twitter.com/owensbfrederick/status/634180163253702656.
Though I cannot personally verify Funky Cops and GI Joe Sigma 6 for whatever name he actually used in those, it would appear that “F.B. Owens” should be the primary identifier for his anime work, but if you feel “Frederick” is more appropriate go right ahead.

XVI. Lance J. Holt (person#27573) (more complete/base name) and Lance Holt (person#31018) (simplified)
As demonstrated by the resume on his official website, he has undergone roles where he has been credited as both names (ex. Hoodwinked 2 = “Lance Holt”, and Lance J. Holt’s known ties with New Generation Pictures should make it clear that the “Lance Holt” credited to Hakan in Super Street Fighter IV is also him).

XVII. Jason C. Miller (person#50208) (main name), Jason Miller (person#13719) (variant), C.J. Mills (person#54985) (alias), and Jaosn C. Miller (person#61024) (erroneous spelling)
Roles credited all these names appear on his resume (ex. Stephen from Read or Die (OVA/Movie) is credited to “Jason Miller”, Pilot in ROD -The TV- to “J.C. Miller”, various roles in Hellsing to "Jason C. Miller", and Yamamoto in Rozen Maiden to “C.J. Mills”), and “Jaosn” is just what it is: wrong.

XVIII. Anthony Salerno (person#4295) (regular name) and Tony Salerno (person#48985) (nickname)
“Tony” is short for “Anthony”, and there is particularly demonstrated in 4Kids’ adaptation of Winx Club, where “Tony Salerno” is credited for voice direction and additional voices in Season 1, but “Anthony Salerno” is credited for those exact roles in Season 2.

XIX. Kayzie Rogers (person#4157) (real name) and Jamie Peacock (person#56130) (alias)
As with the recently merged-up Jimmy Zoppi, Rogers went by a new alias when she continued with Pokemon after 4Kids dropped the license. Just as Jimmy Zoppi became “Billy Beach”, Kayzie Rogers became “Jamie Peacock.”
A number of years ago, Rogers joined Bulbapedia to clarify some details (note the addition of Bellossom) and, on the talk page of her own article, specifically referred to when she was “not allowed to do the show”, referring to when “Jamie Peacock” was taken off the credits about 6 episodes into Season 10 (Diamond and Pearl) and benched for the rest of the season due to violating the “2-year wait” contract for 4Kids actors and Pokemon. This was in response to whether or not she was still Wobbuffet (her main recurring role in the series) during that time, and she assured the community that archive clips of her (from the 4Kids seasons) were used before she came back.
Since then, “Jamie Peacock” lives on in the credits of Super Smash Bros. Brawl and Smash 4 Wii U / 3DS, and in the latter Bellossom (another Rogers-voiced Pokemon) was the only Pokemon voiced by her.

XX. Roger Craig Smith (person#66006) (real name) and Bill Lemas (person#67714) (alias)
As with Jessica Straus on the previous page, when prompted with a Zegapain-related that included his handle, Roger Craig Smith also replied. This is because he voiced the male lead Kyo Sogoru, in what I can only describe as “the voice that would be Sonic the Hedgehog.”

Osamu Sakuta (correct spelling)

Yes. I just have to create a new entry by adding a cast or staff credit with the correct spelling (in this case I re-entered the Inuyasha cast credit) before I can merge them.

Any proof that Shirato is correct? I gave a look at some of the official US credits he has appeared in, and in more recent titles (Eureka Seven Ao, Space Dandy, Steins;Gate) he is rather consistently listed as "Shiroto". Only "Shirato" I could find was in Ouran High School Host Club. I'm more inclined to believe his name is "Shiroto", and that it was officially provided (since it is a rarer reading of the kanji, it's unlikely to simply be a guess).

I. Scott Rayow (person#86785) (real name) and Scottie Ray (person#3878) (stage name)
Rayow follows Amy Birnbaum on Twitter. This “Amy Birnbaum” is indeed the 4Kids actress because Amy in turn follows her relative Rona Birnbaum (the latter is credited in the final arc of Yu-Gi-Oh! Duel Monsters [Memory World/Dawn of the Duel] with Amy). As both “Ray” and “Rayow” have confirmed work at TAJ Productions (with Valkyrie Profile and Shadow Hearts, respectively), it can be inferred that they know each other from there (potentially through the early seasons of Pokemon recorded there, where only “Ray” was credited).

II. James Barker (person#42608) (full name), Jimi Barker (person#36376) (shortened variant) and Jim E. Jam (person#62408) (alias)
Aside from “Jimi” and “Jim” both being short for “James”, DBZ video game credits have included the following people credited for “audio editor” (and variants thereof): Jim E. Jim (Budokai Tenkaichi series), Jimmy Jam Barker (Burst Limt), and Jimmy Barker (Shin Budokai – Another Road, Harukanaru Densetsu). “James Barker” was also credited for One Piece: Unlimited Adventure for the same role. All of the above games were done by Okratron 5000.

III. Chris Smith (voice actor) (person#6261) (real name), Steve Heiser (person#129409) (alias) and Chris Corey Smith (person#52752) (variant)
When you take the English cast credits for Pokemon Origins and change all the aliases to their real names (Maggie O’Connor -> Colleen O’Shaughnessey, Johanna Luis -> Dorothy Fahn, Taylor Henry -> Jamieson Price, B.J. Oakie -> Joe Ochman, David Keefir -> Keith Silverstein), as well as the pre-existing evidence surrounding Heiser, it becomes clear that the English cast is credited alphabetically by first name (first down the left column, then the right), and “Steve Heiser” fits this mold as “Chris Smith” (note: despite his full first name being “Christoper”, the fact that “Chris” is his usual credit name is why he fits between “Bryce” and “Christine”).
“Chris Corey Smith” is, of course, “Chris Smith” with his middle name in tow.

IV. James Corrigall (person#5249) (correct spelling) and James Corrigal (person#20725) (incorrect spelling)
His LinkedIn

V. Dan Green (person#4408) (correct spelling) and Dan Greene (person#102036) (variation)
“Dan Greene” was used instead of “Dan Green” for the final arc of Yu-Gi-Oh! Duel Monsters (Memory World/Dawn of the Duel)

VI. Sam Regal (person#14772) (main name), Sam Riegal (person#99850) (variation) and Sam Reigel (person#66531) (misspelling)
“Sam Riegal” appeared in the credits for the first episode of Yu-Gi-Oh!, as seen here. The last name changed to “Regal” starting in episode 2. “Reigel”, meanwhile is a misspelling of his actual last name: Riegel.

VII. Greg Abbey (person#76403) (correct spelling) and Greg Abby (person#75993) (misspelling)
Verifiable via social media

VIII. Jonathan Todd Ross (person#30327) (full name) and J. T. Ross (person#22591) (abbreviated variation)
“J. T. Ross” (or rather “J.T. Ross”) appears in the Battle City arc credits of Yu-Gi-Oh!, listed for his best-known voiceover role of Marik Ishtar.

IX. John Angier (person#17011) (correct spelling) and John Angler () (incorrect spelling)
His website confirms that “Angier” is correct despite the use of “Angler” in productions ranging from Mewtwo Returns to later seasons of Yu-Gi-Oh

X. Dominick Barbera (person#56412) (correct spelling) and Dominic Barbera (person#128058) (incorrect spelling)
His LinkedIn

XI. Bill Rogers (person#1439) (correct spelling), Bil Rogers (person#56074) (incorrect spelling) and Bill Rodgers (person#72599) (incorrect spelling)
Fairly straightforward. The latter erroneous spelling is from a Pokemon production and was therefore likely to have been confused with Rodger Parsons, as #72600 was Emily Jenness.

XII. Kayzie Rogers (person#4157) (correct spelling) and Kayzie Rodgers (person#113588) (incorrect spelling)
Same “Rogers/Rodgers” problem as above.

XIII. Rodger Parsons (person#56131) (correct spelling) and Radger Parsons (person#56908) (incorrect spelling)
Verifiable via his website, VODirect.

XIV. Debra Jean Rogers (person#14674) (main name) and Debbie Rodgers (person#3041) (variant)
“Debbie Rogers” has been utilized by Debra and has been merged into her profile accordingly. “Rodgers” is, as mentioned a variant. (NOTE: There is also a voice actress in the San Francisco area named Debbie Rogers [who also used both “Rogers” and “Rodgers” spellings] known for stuff like Zone of the Enders (video game), Soul Calibur II and Virtua Quest. None of the anime entries, however, are her).

XV. Stephanie Sheh (person#3419) (correct spelling), Stephanie Shea (person#31213) (incorrect spelling), and Stephine Sheh (person#113589) (incorrect spelling)
Shea, and despite the ANN article for Cyborg 009 vs. Devilman that links to “Shea’s” page, it was still Sheh who voiced her.
Then’s there the other one that’s just missing an ‘a’ and jumbles the ‘i’ and ‘n’.

XVI. Jamie McGonnigal (person#2548) (correct spelling) and Jami McGonnigal (person#18683) (incorrect spelling)
Verifiable via social media.

XVII. Bill Jenkins (person#35887) (correct spelling) and Bill Jenkens (person#53363) (incorrect spelling)
As demonstrated by this article.

XVIII. Gerri Sorrells (person#107950) (correct spelling) and Jerry Sorrells (person#79778) (incorrect spelling)
As seen in her article in The Japan Times, this female English-speaking actress living in Japan’s first name is “Gerri.”

XIX. Richard Nieskens (person#116483) (correct spelling) and Richard Nieskins (person#107941) (incorrect spelling)
As seen on the website for “Dick” Nieskens.

XX. Mary Elizabeth McGlynn (person#1419) (real name), Mary Elizabeth (person#101820) (variant) and Abbey Normle (person#80756) (alias)
“Mary Elizabeth” is credited to miscellaneous voices in the video game Saints Row IV. One would find this suspicious and possibly indicative of a new, different actress… if not for the fact that McGlynn was credited by her full name in the previous title in the series for the same kind of role.
“Abbey Normle” has appeared in two video games with two more spellings (“Abby Normle” and “Abbie Normal”). One of these two games is Street Fighter X Tekken in the role of Nina Williams, and despite never once being credited by her real name, McGlynn has not let her involvement in the franchise be a complete secret.

Tetsu Kimura and Satoshi Kimura are the same person. He doesn't have a Japanese Wikipedia entry, but according to his @wiki entry, "Tetsu" is the correct reading of his given name.

Marc Wihlem (person#102732) and Marc Wilhelm (person#139290) are actually the same person, the correct spelling is Wilhelm (source)
So, it would be great to merge the two.

Also, Jean-Marc Montalto (person#065078) and Jean-Marco Montalto (person#88967) are the same person, the correct first name being Jean-Marco (source)
Again, a merging of the 2 people looks necessary.
