東潤一 has two entries,

Azuma, Junichi (#1655)
Higashi, Junichi (#89)

東 could be spelled either Azuma or Higashi. However, based on Hiragana shown here I'd say the correct spelling is Higashi (ひがし), and all credits in #1655 should be merged into #89.

It's not hard to see the correlation between the two entries; "both" work on Ai Yori Aoshi, Escaflowne, Gundam, and City Hunter. The "two" are actually the same person, anyway. Razz

EDIT: Fixed; this one made me nervous. Anime dazed

池口和彦 has two entries, "Kazuhiko Ikeguchi" (#182, correct) and "Kazuhiko Ikekuchi" (#14062, misspelled).

垂木勉 (タルキツトム) has two entries, "Tsutomu Taruki" (#6602, correct) and "Tsutomu Naruki" (#10912, misspelled).

EDIT: According to this page, he changed the spelling of his last name from Taruki to Tareki (as on his official page) but kept kanji the same (垂木).

EDIT2: Fixed.

In addition to different spelling, I've seen two versions of his name in kanji: 望月智充 and 望月智光. Most websites (>80%) list his name as Tomomi MOCHIZUKI and 望月智充, however, the famous and well-respected Nausicaa.net listed his name as 望月智光.

EDIT: Yoshihisa ARAKAWA: unlikely but unable to confirm. Shinsuke TERASAWA or TERAZAWA: unable to confirm; can't find any furigana or romaji, since both can be 寺沢. The only thing could be confirmed is that his full name is 寺沢伸介. Others: fixed but with minor errors.

Neither Hiro Oikawa (#19156, as in Gatchaman F) nor Hiroo Oyokawa (#3561, as in A Dog of Flanders) is correct. The correct name is Hiroo (ヒロオ, or 広夫 as in real name) Oikawa (及川), who passed away on September 25, 1995 at the age of 60.

Exists correctly as: Amenomori, Masashi (#3562)

and incorrectly as: Amenomori, Mashashi (#4794)

Not really a "duplicate entry", but similar:

Yuuki MATSUOKA (#4683)

and

Yuki MATSUOKA (#5683)

have similar names, but so far as I can tell, they are two different people. The former is "staff", specialising in audio, whereas the latter is "cast". However, the latter has been incorrectly (I believe) credited with

    "(1992) Genesis Survivor Gaiarth (OAV) : Audio Director (Ep 3)"
This should, I believe, have been credited to Matsuoka Yuuki, not Matsuoka Yuki.
