I could never recall another instance in which a manga was debuted in the U.S. before Japan could get it...
Could this manga of Clannad be the first one concerning this?

If you by the definition of manga as meaning "comic" in Japanese and this means comics in general, then there have been so many manga that were released in the US before Japan.

Aoi House comes to mind. It was originally a webcomic in 2005 and was later published as a 4 koma in Newtype USA. To my knowledge it was never released in Japan, which is understandable because its quite boring.
