Had forgot about this one, but unlike other cases where the disturbing content is in there just for effect, this one is actually critical to the plot; the character responsible dwells on it for the next several episodes and his actions and behavior are affected by it.
And Deltakiral: That episode played at the normal time in the U.S. but it was edited a bit. (IIRC it was the only episode in the first season that wasn't broadcast uncut.)

While Elfen Lied certainly had some disturbing scenes, particularly Nana's graphic dismemberment in episode 4, I have yet to find a show more disturbing than the end of Shadow Star Narutaru.
In the last few episodes of Shadow Star Narutaru, you have girls molesting a classmate by sticking a test tube full of worms in her, said girl going psychotic and murdering her parents with her new Dragon, and then killing most of the girls who molested her (I think the last only suffered broken limbs and severe trauma thanks to Shiina's intervention), the most disturbing being the leader girl, in which the dragon kills her brother (which seemed to be getting ready to sleep with her) and then injects worms into her and kills her in a particularly gruesome way related to the way she molested the earlier girl
However, I am currently watching Higurashi no Naku Koro Ni (I've seen the first two episodes so far) and this series could come close to matching Elfen Lied and Narutaru.

My first glance at Urotsukidoji - Legend of the Overfiend was unforgettable to say the least! I hadn't seen much anime before and I had NEVER seen anything like some of those scenes featured there. Poor girls being raped by tentaclemonsters from parallel dimensions until they explode weren't exactly something I was familiar with by the first time I saw it .

I just watched episodes 3 and 4 of Higurashi no Naku Koro Ni. And if this show keeps things up, its well on its way to being one of the most disturbing shows ever.

Not going to disagree here. I don't know, maybe it was because BB was the first hentai series I watched in its entirety. Maybe if I go back and watch the scene again it might be less disturbing for me.

Only two anime that I can think of really did it for me. The first was Makai Tensho (Ninja Ressurrection out here)...the whole damn thing was just sick, especially the rape scene and the scene where the two kids get chopped up and their heads shown to Shiro Amakusa to turn him to the dark side. The other was Violence Jack which was just filled to the brink with rape scenes and needless violence. Again, the scene that killed it for me is where the little kids get slaughtered by the invading gangs. Any thing that has little kids getting killed gets an automatic F in my book. That's just sick.

I watched the first ten seconds of episode 1, and I already concur on that fact. I decided to drop it after watching the first scene; usually disturbing scenes equate to depressing moments, and I can't stomach another one of those.

Oh, of course--it is a very relevant scene for the plot, and Koi Kaze wouldn't have as much power or believability without it. (*makes plans to rewatch the series and read the manga*)

Well, Bible Black was the first hentai that I watched completely, but I had had enough exposure to non-animated porn that I didn't find it disturbing. (Then again, maybe normal people *should* be disturbed by it...) Incidentally, the few parts of BB that did disturb me were some of the blood-ritual stuff and the "water torture" incident.

I'd have to agree with the "you're not my mother" *gorey obliteration* scene from Elfen Lied.
I'm gonna go out on a limb and say the scene from the last episode of Excel Saga where everybody gets raunchy over each other was disturbing in places...but hilarious nonetheless .

I was going to mention that same episode from the first season of GITS:SAC. The second season also had a less gruesome but just as shocking scene: the televised beheading of all members of the Individual Eleven (except for Kuze). For me it was one of those "WTF???" scenes.
Another disturbing scene for me was in NGE when Eva Unit 01 was freed from its controls and Shinji Ikari's synch ratio reached 400% and the Eva was eating the remains of the angel it had defeated. I had absolutely no clue that was going to take place until after it began happening.

I have to agree with a previous poster in that any scene where a women is getting raped just plain messes me up. Usually I have to pause the movie I am watching, and take a couple minutes to calm down. Then I have to emotionally brace myself. On one hand it totally disturbs me, and on the other I feal that I have to see it atleast once.
Other things that hit me hard are cruelty to animals, and to children. Though I seem to be able to stomach them, but show me a girl getting raped or even insinuate it, and I get all sick inside. I suppose that is the point of such scenes to get you emotionally involved.
In the series Now, and Then Here, and Now. There is a scenewhere the character Sara is led to a room, and while they do not show the rape. Well that scene had me stop the DVD, and go take a shower it had my blood boiling. A credit to the series I suppose it definately got me more involved in the plight of the characters.

Not surprised that NTHT finally got mentioned. That whole series is disturbing, but that isn't the particular scene I would have picked. I would have instead gone with the scene at the end of episode 3 (I think) where Sara's been left in the cell after being taken out the first time, and when the beat-up Shu is deposited back in the cell he gets a hazy look at her sitting there dazed with her clothing torn open. Doesn't actually show anything but the implication is so powerful and heartbreaking in so many ways that I could easily see how it could get a person worked up. (Thank goodness for that soothing closer music! Situationally speaking, it's maybe the best closer ever.) It's also one of the rare times in anime that I'm glad that the nudity was left indistinct because that would have been too much.
The other NTHT scene I would pick is, of course, the one a later episode where Sara hits herself in the stomach with the rock when Shu stops her from committing suicide. Given what that action means and why it's happening, I don't see how a sensitive person could watch that without at least cringing.

You should keep watching, its not really depressing, just seriously scary. I can't remember the last time an anime made genuine chills run down my spine. Also to get this much awsome plot progresion in a mere 4 episodes is just amazing.

This isn't a scene from anime, but rather manga, but it still got to me and I think it's gotten to alot of other people if they've read it. In Ichi the killer, when they had one of Jijii's gang members tied up (can't remember his name) to a chair, and then Kakihara came in all smiling and presented a flyer that showed of ways they pierce penises. And Kakihara took a pair of scissors and just went *click* and the guy's penis was in half and blood was just pouring out. As a guy, that made me hurt and kinda bring my legs together for a while.
