fist of the north star is probably the quintessential PA manga. eden is new and quite good, with great art and an intriguing story.

Nausicaa of the Valley of the Wind is an awesome post-apocalyptic manga. Not as grusome as Fist of the North Star, but all of the story and art is by Hayao Miyazaki, so you KNOW that its good.

If you don't mind shoujo, BASARA by Tamura Yumi is a great manga with post-apocalyptic/fantasy-like setting.
Tamura's current work, 7SEEDS has even more post-apocalyptic feeling to it. It's more survival/horror type of story. But I'm not sure if it's available in the U.S.

Akira anyone? Sure, it isn't stricktly post-apocalyptic (at least the first half isn't), but it's better than anything else mentioned with the exception of Nausicaa (another timeless classic that should be manditory reading for manga fans.)
Legend of Mother Sarah is another post-apocalyptic manga from Katsuhiro Otomo that's quite good. Darkhorse ony released thirty or so issues over here, but I've been able to find scans up to about volume 10 and I'd recommend it to anyone who likes his prievious work.

The first two volumes of Eden are now available from Dark Horse.
Mankind has been devestated by a horrible disease and only a very small percentage (1% or less) of the population developed immunity and survived. This is the story of a young boy and his robot companion as they try to survive. You can get a bit more detail in this weeks "Right Turn Only" column.
Trigun/Trigun: Maximum technically isn't post-Apolyptic but it has a similar feel and the situation humanity in is similar.
