I have a seiyuu in an OAV whos name is romanized Terashita Yukiko. The problem is that the database already contains a person with the same name, though her name has different kanji. What to do?
Existing Yukiko: 寺下 由紀子
'My' Yukiko: 寺下 由希子

The first thing is to make sure they are really different. Maybe she just changed a kanji in her name. It happens. Then find what makes them different. For example we have 2 "Osamu Tezuka":
Osamu Tezuka (The God of Manga)
Osamu Tezuka (composer)
In the same way we need a little comment to differentiate between those 2 Yukiko Terashita. Since both are seiyuu, maybe the best is to differentiate with their year of birth. Also, we need to know who is the "main" Yukiko Terashita, i.e. who is best known. Then add the credit under the existing name and tell us which is which so we can do the split.
