On hand, I've never seen him do it, but unfortunately I haven't read enough of the manga to give you a straight enough answer.

I consider the whole idea of 'choice' kind of a fanmade (or anime created) thing.

His feelings for Kikyou are not the same as his for Kagome. He loved Kikyou, but things are different now and have been for a very long time. He promised to protect her, and yeah if he had to die with her he would because of past guilt. However, his romantic feelings are Kagome's and he's referred to the two of them directly as being in a relationship as well as vowing that he'd always be with her.

He would die with Kikyou if he had to, but he wants to live for Kagome, which is really a much more powerful thing. It's a traditional case of duty vs. desire. They've always been kind of a metaphor of life and death in his life, and of course he's chosen life.

In volume eighteen he almost 'breaks up' (his words) with Kagome because he feels like he can't stay with her while being indebted to Kikyou. He says he will protect Kikyou there, so yes I suppose he does. However, in the same arc he reveals that what he wants to do is be with Kagome (you make me happy, you're the first person I've ever trusted, various sentiments), but he feels as if he can't because of Kikyou dying after him. But he never says 'I'll protect you over Kagome,' and says he'll protect Kagome many times and also says he would die for Kagome at more than one point. No he is not two-timing like many people would like to think. He has two women in his life, but their roles are very different. So it becomes a case of reading between the lines.

If he truly valued protecting Kikyou over Kagome, all dialogue aside- he certainly wouldn't leave her on her own 90% of the time. Kind of hard to protect someone that way. She's shown that she has a great deal of trouble fending for herself as well, and by ratio of appearance, has been rescued and saved more often than her reincarnation, so the mindset of 'Kagome needs more protection' couldn't even be the case. Plus, even if she (Kagome) did need protection, the rest of the group could do it.

All that aside though- spoiler[In volume forty-seven of the manga Kikyou died fighting Naraku. For real this time. No false deaths of cliffs. He also promised to protect Kagome with his life, in a very romantic scene (They almost kissed. Damn you Souta!) in volume fifty. ]

spoiler[ Since Kikyo Is now dead, I guess Inuyasha won't have to choose anymore, huh?] but that aside, when Kikyo was first resurrected, he would go to amazing lengths to protect her (I think). Eventually he mellowed out and tried to protect both when possible, or would go after the one in greater danger (I think). But listen to the kazenoyume guy above me, he's got the right idea.

Oh yeah he did, but even right after her resurrection, he went after Kagome at the drop of a hat. When Kikyou tried to take him to hell in her first appearance after her resurrection, he immediately leapt away from her and broke out of her spell and 'rescued her' when he just noticed Kagome was there. At that time she wasn't even in any real danger. Kikyou's shinidimachuu (soul catchers) were loosely wrapped around her, but all they were really doing was making her invisible to IY. His feelings for them aside, I think IY would subconsciously always protect Kagome first and foremost (in the early series instance where he almost had to leave her, he planned to leave her safely in her era) simply because she is alive. Deep down I think IY always knew that there was no way to save Kikyou, just make sure she was able to die in the way she wanted and having avenged against Naraku.
