Oh, we're at the 'O' :
(correct entry/entries are marked in bold)

*Oh production and Oh Productions
Source : according to the website, it should be Oh!Production, but the first entry is OK

*Once TV, OnceTV and Canal Once (IPN)
Source : the Once TV website and the Wikipedia entry of it

*ORF 1 and ORF1
Source : the ORF website

*Osaka Anime Word (typo) and Osaka Anime World

This is what I've found as duplicates starting with 'P' :
(correct entry/entries are marked in bold)

*Pang Studios and PangStudios
Source : according to the website of the Swedish Film Commission, Pang Studios is now merged with Resource Film and Audio Building to Morgankane. This Pang Studios is the company with the duplicate entries.

*Pierrot, Pierrot Project, Studio Piero and Studio Pierrot
Noticed and correctly proven by Dormcat in a previous post
Dormcat, what about Pierrot D.A.R, Studio Pierrot Fukuoka and Studio Pierrot Lab ?

*Pink Pineapple and PinkPineapple
Source : the PinkPineapple website

*Please ! and Please!
Source : in the Onegai teacher manga, his/its name is written as Please! (without space)
Also noticed by Sarki-Kun

*Point.360, VDI - Point.360, VDI Media West, VDI Multimedia West and VDI West Multimedia
Source : There is this one link on the page of VDI Multimedia West, which points to Point.360. On that website, I can't find any reference to VDI whatsoever. However, I found another link which states that .360 was formerly known as VDI Mulitmedia.

*Pony Canyon, Pony Canyon Enterprises and Pony Crayon
Also noticed by Sarki-Kun
Source : the Pony Canyon website

*Production I.G and Production I.G.
Also noticed by Sarki-Kun
Source : the Production I.G website

*Productio I.G USA (typo), Production I.G. USA and Prouction I.G USA (another typo)
Also noticed by Sarki-Kun
Source : by analogy with the previous duplicate (this is the US affiliate), it should be "Production I.G", but I don't think there's a need to create a new entry.

For the following duplicates, it's not clear what the official name should be :
*PAC and Pacific Animation Corporation
*Pak Pro, Pak Production, PAKPRO and Paku Production
*Princess Comics and Princess Magazine
*Production AI and Production Al
Also noticed by Sarki-Kun
*Project EVA and Project Eva.
Also noticed by Sarki-Kun
*Project Team Argos and Project Team ARgus
*Pug Point and Pug Point Japan

And here's a very short one :
(correct entry/entries are marked in bold)

*Q Tech, Q-Tec and Qtec
Source : the Q-Tec website
Already noticed and proven correct by Samurai CDZ in this post

*Q.T. Lever and Q.T: Lever
Source : according to their website, the name should be qt.lever, which is nevertheless quite different from the two existing companies. If you don't want to create a new entry, keep the first one (the double colon is not appropriate)

The Japanese Wikipedia entry states that it is "Maki Production".

The Web site states "Monkey Boy Productions", though they should be contacted to confirm that they are the same studio—I just E-mailed them. Note that the existence of the person Monkey Boy POST, credited by CrystalAcids.com with numerous instances of "DVD Post Production". This is probably the same company. Note also that all three entries feature mostly (exclusively?) ADV titles.

Here are the duplicates starting with 'U'
(correct entry/entries are marked in bold)

*UCV and UCV Television
Source : the UCV TV website
meitantei's request to change UCV to UCV Ueda Cable Vision is first difficult and second not really appropriate : there are no entries for UCV Ueda Cable Vision as a broadcaster yet. Better is when they appear, to enter a new company with that name.

*UFO Table, ufotable and ufotable zippers
Noticed and commented by Dormcat
Source : according to the website, it should be ufotable.inc but we can keep ufotable

*Universum Film and Universum-Films
Source : officially, it should be Universum Film GmbH, therefor, you can keep Universum Film

*Urban Vision and Urban Vision Entertainment
Source : officially it should be Urban Vision Entertainment, Inc., but Urban Vision Entertainment is OK

And again some duplicates without source or name proof :
*UFO Group and UFO Group (Warner Music Japan) (both entries are empty ???)
*U-ni animation, Uni Animation and Uni-Animation
*Usagi-ya and Usagiya

Well, I'm a long-time fan, and was one of the original subscribers to Mixxine. Originally, they just changed the name that they used with the public (the "dba", or "doing business as"), but since the incorporation was done by different people, there may have been more to it than that.
