Crazy to think that this more or less only happened because Hololive was way more lax with game selection in those early days, and it wouldn't have been long after that that Konami would've just said no and squashed the whole thing

Nay, the real reason why it happened is as simple as it can be - Kojima not even hiding that he's Pekora fan, at least since 2023 (before she visited KojiPro studio last year) - and when you get into Kojima fan zone, that means he obviously bring you into the game one way or another. |
And Kojima wouldn't have been a fan had Pekora not played the MGS games, which wouldn't have happened even a short while later when Hololive began to get stricter on what games could be played with permission from the rights holders. Which is what my post was about

You kind of have to be more selective when you get really big because then you become a lot more liable if you do something wrong and if a game company says no then you're out of content. Hololive has had Konami perms for around 4-5 years now though so they're assumedly on good terms.

Your timeline is off.
Hololive started their "only stream games with publisher permission" following Capcom almost nuking Mio's channel with copyright strikes in 2020.
Kojima's first tweets about watching Pekora play MGS3 are from her 2023 streams of the game, which have the (paraphrased) "this stream is under license with Konami Digital Entertainment" disclaimer in the description.
