Some Japanese names from the report's top entries:- [move/purge] Takahashi IIJIMA (source: English Wikipedia, and even misread the correct name there) is a contribution error for 飯島 敬 = Takashi IIJIMA (Wikipedia page of anime)
- [move/purge] Takahashi FUJII (source: animenfo, and even misread the correct name there) is a contribution error for 藤井 隆 = Takashi FUJII which itself is a split case (seiyuu, animator).
- [move/purge] Takahashi NOBUYA appears to be a kanji misreading duplicate of Shinya TAKAHASHI (高橋 しんや at ANN but 高橋信也 at Wikipedia which can be read both ways) as Shin'ya is already credited for Character Design in this anime. Wikipedia doesn't credit him for 作画監督 as well, so this credit for Charlotte (TV) isn't really backed up so far.
- [move/purge] Tanaka JSSEI is a spelling and reverse name order duplicate of Issei TANAKA which itself is a kanji reading error duplicate (purge request [done]) of Kazunari TANAKA (seiyuu) which already has been split from a staff with the same name... phew.
- [move/purge] Sato TATEISHI is a kanji misreading duplicate of 立石 聖 = Kiyoshi TATEISHI (as error-flagged by braves, 35 ENAMDICT readings for 聖 including both Sato and Kiyoshi).
- [move/purge] Kobayashi YASUKO is a reverse name order duplicate of 小林 靖子 = Yasuko KOBAYASHI, the script writer of Shakugan no Shana for which Banjō no Carmel-san is a special / fake promo.
- [move/purge] Nakamura YUUICHI is a reverse name order duplicate of 中村 悠一 = Yūichi NAKAMURA (seiyuu), "both" voice acting in "Sayonara Zetsubō" releases.
- [move/purge] Nakamura HIDETOSHI is a reverse name order duplicate of 中村 秀利 = Hidetoshi NAKAMURA, with the credit for "ダッシュ！四駆郎" mentioned on his Wikipedia page (1989).
- [move/purge] Hayashi HIKARU is a reverse name order duplicate of 林 光 = Hikaru HAYASHI.
- [rename] Saito YOSHIHIRO apparently got this name order because his Japanese name is written in Katakana (サイトウ ヨシヒロ). I would still assume SAITOU to be his family name. No reverse version exists at ANN, so this person might be renamed.
- [rename] Aoki TAKAMASA (with picture and official website) is a wrong name order version of 青木 孝允 which should be renamed to AOKI, Takamasa (as this entry doesn't exist yet).
- [rename] Aoki SATOSHI is 青木 聡 which should be renamed to AOKI, Satoshi (animator) to disambiguate him from Satoshi AOKI (a music producer).
- [unclear] Ishii AKAKURA is 赤倉 一久, so the surname seems to be correct but "Ishii" is no valid reading for 一久. ANN has 1 一久 person = Ikkyuu JUKU but this might not be enough to derive the given name of 赤倉 一久 from it. ENAMDICT would allow 8 readings for 一久 .
For the calculation of the difference between surname appearances and given name appearances, the report should rather not count person entries without credits (and visualize them somehow differently in the report).
It might also be worth thinking about some flagging mechanism for "valid exceptions". "Sato" is a valid given name in a few cases; Kaneko IWASAKI is a valid exception of "Kaneko" normally being a surname.

"コング 桑田" (Kong Kuwata) actually is a synonym of 桑田 敬信 (Kuwata ???, no reading given thus I can't add it as "Birth Name"), as per his Wikipedia page.
