This article contains a spoiler. I watched the entire anime not knowing that there is a relationship between Mikasa and Levi, but Levi's last name as mentioned in this article suggests that they are actually related.

Apparently, they say his last name in the Funimation dub: animenewsnetwork.com/review/attack-on-titan/bd-dvd-part-2/.80046

Even in the manga, it's not exactly treated as a major plot revelation the first time someone says his name... from what I remember, somebody just casually calls him "Levi Ackerman".

I think this forum topic is more spoilery than the actual post. It's not uncommon for people to have the same surname, whilst not being related. I certainly didn't look at it and think there was a relationship between the two.

Ironically, Levis are also a popular pair of jeans (you have to pull your jeans down to sit on the loo).
