I find that cover certainly intriguing--does something go on with Sumire or is that simply her in costume for a stage play?

That is not Sumire, but Ayame.
In the 1st game, Ayame gets brainwashed by the main villain, Sa'tan, and awakens as his demonic minion (she also gets a new sexy leather outfit). She dies when defeated in battle, but is resurrected as an angel, Michael (the form you see here.) And yes, Sa'tan is actually The Christian Satan, come to unleash the apocalypse on earth.
