Hitomi Amemiya is a misreading of Hitomi Amamiya. Her official website confirms this in the logo and profile, and the Writers Guild listing backs up this reading as well. Pretty conclusive proof, so I'll go ahead with adding the credits to Amamiya and purging Amemiya.
